Dan
Tobin, M.D.
Dan Tobin,
M.D., is the CEO of both CAICC and Care
Support of America. Dr. Tobin serves as an Adjunct Assistant
Professor of Psychiatry (Health Psychology) at Dartmouth Medical
School and a nationally recognized leader in integrating care
coordination and health counseling within health plans, hospital
systems and doctors' offices. He is the author of books, articles
and training manuals focused on improving the quality of life
for patients and caregivers facing advancing chronic illness.

Joe
Englehardt, Ph.D.
Joe Englehardt,
Ph.D, is Director of Health Services Research for the Center for
Advanced Illness Coordinated Care. Dr. Englehardt is a social
worker who has served as principal or co-principal investigator
on 15 health services research and development studies in the
Department of Veteran Affairs. He is Research Professional in
the School of Social Welfare, University at Albany. Dr. Englehardt
has authored publications and technical reports on care management
and health care cost outcomes.

Jian
Gao, Ph.D.
Jian Gao,
Ph.D., is the Chief Economist and Statistician for the Veterans
Affairs Healthcare Network Upstate New York. He holds a Masters
degree in Management Science and a Ph.D. in Economics. As the
Chief Economist, Gao designed the resources allocation system
that has been adopted by six medical centers in New York for their
efficiency evaluation and budgeting since 2000 With more than
20 years of experience in various factories and companies, Gao
is devoted to bridging the gap between healthcare managers and
advanced economic/statistical tools His research interests include
healthcare policy design and evaluation, managerial efficiency
improvement, and resources allocation.

Joe
O'Donnell, M.D.
Joe O'Donnell,
M.D., is the Chief of Medical Oncology at the White River Junction
VA Medical Center where he has practiced medicine since 1978.
He is Senior Advising Dean and Director of Community Programs
at Dartmouth Medical School, reflecting his interests in human
values aspects of medicine, prevention ethical issues and the
medical humanities. He has worked vigorously to address the issues
of substance abuse, obesity, and to promote healthy lifestyles.
