Chronic Illness Care Coordination

The Center for Advanced Illness Coordinated Care (CAICC) focuses on the practical application of health services research in the area of adult and pediatric care coordination and health counseling. CAICC has overseen research in a wide range of healthcare settings (multi-site healthcare systems, regional hospitals, homecare and doctors' offices). This research has been supported by the Robert Wood Johnson Foundation, The Nathan Cummings Foundation, Fan Fox Samuels Foundation and The Garfield Foundation/Kaiser Permanente.

CAICC developed the Advanced Illness Coordinated Care (AICC) training and has consulted with numerous companies including Blue Cross Blue Shield of Montana, Massachusetts and Western NY, Oxford and Humana.
